Lake County Residents Survey

Lake County Mobilizing for Action through Planning and Partnerships (MAPP) steering committee is a community partnership of individuals and groups working to improve the quality of life for all residents of Lake County. This active partnership looks at the county's challenges related to community health and works to find solutions. With your help, the committee will work to make life better in Lake County.

All residents of Lake County are invited to take the following survey in order to assist the MAPP steering committee identify community health challenges. Please note that the survey results are gathered anonymously and are not linked to any names or addresses. The survey deadline is October 21, 2011. Any questions can be directed to Jesus Ruiz of the Lake County Health Department at (847)377-8118 or at HealthMAPP@lakecountyil.gov.

All parents are invited to schedule conferences with teachers regardless of student progress, and to especially schedule a conference if their child received a grade that warrants your concern.
